Senior Clinical PharmacistI am delighted to partner with a leading, expanding Hospital in Dublin to appoint a Senior Clinical Pharmacist.Sample of Duties:Participate in ward pharmacy services, providing advice to ensure safe + cost-effective prescribing.Undertake Medicines reconciliation for patients admitted to hospital.Monitor in-patient prescriptions to ensure clinical accuracy and cost-effective prescribing.Assess the patient’s response to therapy, interpret blood results, taking action as necessary e.g. discussing with clinicians / altering medications.Advise on therapeutic drug monitoring.Monitor and report any Adverse Drug Reactions experienced by patients.Take responsibility for checking and final release of aseptic products to include cytotoxic chemotherapy and intravenous additives.Minimum requirements:PSI registrationA minimum of 3 years Hospital Pharmacist experience.Any experience in Oncology services and/or Aseptic Processing is an advantage.For further information on this role, please forward your CV to Seán.Your CV will never be shared with any 3rd party without your express permission.
