The Senior Coordinator prioritizes multiple tasks simultaneously, ensures timely follow-up on discharge needs, and supports efficient patient throughput while maintaining excellent customer service and attention to detail. Strong organizational skills, adaptability, and the ability to thrive in a demanding healthcare setting are essential.Senior Coordiantor for Case ManagementThe Senior Coordinator for Case Management would work under the direction of the RN Care Managers and Social Workers. This position functions with his/her peers and other care providers for problem solving and facilitating in-patient and post hospitalization care. And coordinate, oversee records and transmit information pertinent to the resource management of patients.Job QualificationsAssociate's degree or High School Diploma and equivalent relevant experience required. Bachelor's degree preferred.Medical assistant or Licensed Practical Nurse (LPN) highly preferred.5-7 years of customer service, Insurance knowledge medical assistance or secretarial experience preferred. Prior experience in a medical setting requiredAbility to organize and utilize work hours effectively and with minimal supervisionMedical terminology is a mustMust be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge. Essential ResponsibilitiesCommunicate information received from payers to utilization review nurse.Transmit continued stay reviews and track authorizationsComplete HENS/PASSRScheduling PCP/follow up appointmentsFaxing and phoning agencies and facilities to assist with discharge referrals and continuity of careAssist with delivery of charity items-clothing/DME/meal cards, etc. and complete transportation application and arrange transportation as needed for patients at dischargeCompleting PCS formsCompiling packets for transportDME: Completes form, get the script, and call liaison to follow up to ensure the patient receives before leaving.Sending referrals through Epic CareLink, Facilities, and IV infusionCalling facilities checking on patient acceptanceFaxing paperwork to facilitiesPrinting documents from EpicProblem Solving and Multi-tasking is a must quality Build relationships with Community Partners *This is a full-time position at Mount Carmel St. Ann's and supports the North Region to include MCNA and MCD.Mount Carmel and all its affiliates are proud to be equal opportunity employers.
