Senior Full Stack Engineer (NestJS / React)

About the ProjectExpert-I is an established teleradiology SaaS platform serving healthcare providers across multiple countries. The system supports complex clinical workflows, high data volumes, and strict regulatory requirements, operating across regions with distinct data residency and compliance constraints.We are looking for a Senior Full Stack Engineer (NestJS / React) to drive end-to-end feature delivery, shape technical architecture across both the front-end and back-end, and ensure scalability, performance, and compliance across our clinical platform.ResponsibilitiesFull-Stack Architecture & Delivery: Design, build, and maintain end-to-end features spanning scalable NestJS backend services and high-performance React client applications.Front-End Workflows & UI: Build interactive, data-dense clinical worklists with advanced filtering, sorting, pagination, and real-time state synchronization using React, TypeScript, TanStack Router, TanStack Query, and Zustand.State Management Architecture: Design clean client-side state models, separating server cache (TanStack Query) from global/transient client state (Zustand) to avoid unnecessary re-renders.Clinical Tools & Editors: Maintain and extend customized rich-text diagnostic reporting editors and integrate web-based medical DICOM viewers into clinical workflows without sacrificing UI responsiveness.Back-End Services & APIs: Design and implement RESTful APIs, business logic, and tenant-isolated microservices with NestJS.Database & Data Modeling: Design and optimize database schemas, handle migrations, and tune complex SQL queries for high-volume transactions.Identity & Multi-Tenancy: Implement and enforce multi-tenant isolation, role-based access control (RBAC), and authentication flows using OIDC / Keycloak across both client and server layers.Media & Document Pipelines: Handle large file uploads, client-side compression, and document export workflows (PDF / DOCX).Testing & Quality Assurance: Ensure high test coverage across the stack using Vitest / React Testing Library / Playwright on the front end, and Jest / Supertest on the back end.Platform & DevOps Collaboration: Collaborate with the DevOps team on containerization, CI/CD pipelines, Kubernetes environments, and cloud infrastructure.Compliance & Security: Ensure all architecture and implementations comply with healthcare data protection standards, including HIPAA and GDPR.Requirements & ExperienceProfessional Experience: 5+ years of professional software engineering experience delivering production-grade web applications across the full stack.Front-End Stack:Advanced proficiency in React 18+ with strict TypeScript.Strong mastery of State Management: Client-side state using Zustand alongside server-state caching/invalidation with TanStack Query.Deep experience with TanStack Router (type-safe routing, search params validation, loaders).Strong UI fundamentals with Tailwind CSS, modern CSS/SASS, and Framer Motion.Experience with modern build setups and monorepos (e.g., Vite, Turborepo, pnpm).Form handling and validation using React Hook Form with Zod or Yup.Back-End Stack:Extensive experience building robust Microservices and APIs with NestJS and TypeScript.Strong relational database design and SQL query performance optimization (PostgreSQL or similar).Experience designing Multi-tenant, Distributed architectures and RESTful integrations.Authentication & Security: Solid working knowledge of OAuth2 / OIDC authentication patterns and identity providers (such as Keycloak).Testing: Hands-on experience with unit, integration, and E2E testing (Vitest, React Testing Library, Playwright, Jest, Supertest).Collaboration: Strong experience working within Agile/Scrum engineering teams alongside product managers and designers.Bonus Qualifications (Nice to Have)Deep experience with rich-text editor internals (TipTap / ProseMirror custom extensions, schemas, and plugins).Experience with medical standards and imaging workflows (DICOM, HL7, web-based DICOM viewers).Experience developing and maintaining internal shared component libraries and design tokens.Familiarity with observability and logging tools (Prometheus, Grafana, or OpenTelemetry).Working SetupHigh ownership and meaningful influence over technical architecture and product roadmap.Close collaboration with senior leadership and core engineering.Work on a large-scale, mission-critical healthcare platform operating internationally.How to ApplyApply via LinkedIn or send your CV and a short introduction to:hr@expert-i.com
