Our mission is to ensure that every client receives personalized attention and support tailored to their healthcare needs.About the RoleThe role involves overseeing the admission process, conducting assessments, and ensuring the delivery of high-quality care to clients at CCF.ResponsibilitiesCCF Clients' Admission Process: The admission process begins with a facility visit or sharing clinical details, including medical reports, with the medical officer. Clients are evaluated based on their current medical condition, and additional information is sought if needed. Pre-admission assessments help analyze healthcare needs, with recommendations for admission or alternative measures provided. If admission criteria are unmet, clients are informed with proper documentation.Initial Assessment & Plan of Care: Within 4 hours of admission, an initial assessment is conducted, and a comprehensive care plan is prepared within 48 hours. The plan includes nursing care, dietary guidelines, rehabilitation goals, planned investigations, counseling, and anticipated barriers to care. Coordination with nutritionists, physiotherapists, and nursing staff ensures a holistic approach.Daily Care of Clients: Clients are monitored daily and on an SOS basis, with input from family, caregivers, and healthcare teams. Care plans are adjusted as needed, and clinical updates are shared with primary clinicians. Notes are documented in SOAP format, adhering to NABH standards. Safety is prioritized through adherence to International Patient Safety Goals (IPSG).Family Meetings: Family meetings are conducted within 48 hours of admission to review care plans and address queries. Regular fortnightly or SOS meetings update progress and discuss advanced care directives. All discussions are documented and shared with family representatives when necessary.Discharge Process: Discharge planning involves a thorough review of clinical progress, milestones, and therapy recommendations. A detailed discharge summary ensures continuity of care at home. In case of complications, appropriate medical interventions are taken.Team Meetings and Additional Roles: Daily CCF team meetings address clinical and operational aspects of care. Medical officers also participate in professional development, quality improvement, and teaching initiatives.
