The role focuses on preventive, acute, and chronic care, with particular emphasis on managing complex medical conditions, developing individualized care plans, and coordinating multidisciplinary healthcare services.The Senior Registrar plays an essential role in improving patient outcomes, reducing avoidable hospital admissions, and promoting continuity of care through evidence-based medical practice, proactive disease management, and close collaboration with healthcare teams.The position requires strong clinical judgment in managing complex cases, appropriate escalation of care, and effective coordination with specialists and multidisciplinary healthcare professionals while ensuring safe, compassionate, and patient-centered care.Key Responsibilities1. Patient Consultation and Clinical Case ManagementConduct comprehensive medical assessments during home visits and virtual consultations.Assess, diagnose, and manage a broad range of acute and chronic Internal Medicine conditions.Manage complex and high-risk patients, including those with cardiac, renal, hepatic, and other complex medical conditions.Identify high-risk patients and initiate timely interventions to prevent complications, hospital admissions, and unnecessary emergency department visits.Develop and implement individualized treatment plans, including medication management, hospital referrals, and lifestyle modifications.Monitor patient progress and adjust treatment plans based on clinical findings and patient outcomes.Exercise advanced clinical judgment when managing complex medical cases.Escalate care appropriately when consultant input or a higher level of care is required.Coordinate referrals and consultations with specialists and multidisciplinary healthcare teams.Participate in multidisciplinary case reviews and contribute to clinical decision-making.Provide patient and caregiver education regarding medical conditions and treatment plans.Utilize telehealth and video conferencing platforms for virtual consultations, assessments, follow-up, and patient education.Ensure accurate, evidence-based, and timely documentation of assessments, diagnoses, and treatment plans.2. Quality Assurance and ComplianceMaintain compliance with applicable healthcare regulations, clinical governance frameworks, and accreditation standards.Practice according to approved clinical guidelines and organizational policies.Participate in quality improvement initiatives and clinical audits.Maintain high standards of clinical quality, safety, professionalism, and ethical practice.Contribute to maintaining a safe and effective home healthcare environment.Support compliance with regulatory requirements and ensure accurate and complete documentation for audits and inspections.3. Professional DevelopmentParticipate in continuing medical education (CME), workshops, conferences, and clinical training programs.Participate in educational sessions, case-based discussions, and clinical teaching activities focused on home-based patient management.Share clinical knowledge and best practices with multidisciplinary team members.Support the development of clinical guidelines, care pathways, educational resources, and service improvement initiatives related to Home Healthcare.Maintain up-to-date knowledge of Internal Medicine, home healthcare, population health, and evidence-based clinical practice.Support clinical education and guidance for medical staff when required.4. Health Information Systems (HIS) and Medical DocumentationUtilize Health Information Systems (HIS), electronic health records, and telemedicine platforms effectively.Ensure complete, accurate, and timely documentation of patient assessments, diagnoses, treatment plans, and follow-up notes.Utilize HIS-generated reports, dashboards, and clinical data to monitor patient outcomes and identify care gaps.Participate in training related to healthcare information systems and platform enhancements.Maintain patient records in compliance with Ministry of Health requirements and organizational standards.5. Patient Safety and SatisfactionApply patient safety principles and clinical risk management practices across all clinical interactions.Identify and report patient safety risks, clinical incidents, and adverse events.Participate in patient safety initiatives, quality improvement projects, and multidisciplinary case reviews.Provide compassionate, ethical, and patient-centered medical care.Take complete medical histories and communicate effectively with patients and families.Clearly explain diagnoses, proposed management plans, and expected outcomes.Address patient concerns promptly and professionally.Contribute to maintaining high patient satisfaction and quality of care.6.
