قدّم وتابع مع أبلاي إيدجRehabilitation Case Manager – Social Work BackgroundTri-Star Health Management Group Inc. is seeking a compassionate, organized, and experienced Rehabilitation Case Manager with a social work background to support individuals and families navigating complex injuries, health challenges, and rehabilitation needs.This position is particularly suited to a social worker who combines strong clinical insight with excellent care coordination skills. The successful candidate will understand how medical, emotional, family, financial, and environmental factors can affect recovery and participation in daily life.Key ResponsibilitiesComplete comprehensive assessments of clients’ rehabilitation, psychosocial, functional, and community-support needs.Develop individualized rehabilitation and treatment plans in collaboration with clients, families, and healthcare professionals.Coordinate services provided by occupational therapists, physiotherapists, psychologists, social workers, speech-language pathologists, rehabilitation support workers, personal support workers, physicians, and other specialists.Provide supportive counselling, advocacy, crisis intervention, and practical assistance to clients and families.Facilitate referrals and help clients access appropriate medical, rehabilitation, mental-health, housing, financial, and community resources.Attend medical appointments, treatment-team meetings, discharge-planning meetings, and case conferences as required.Monitor clients’ progress, identify barriers to recovery, and adjust rehabilitation plans when needs change.Maintain regular communication with clients, families, treatment providers, and other involved parties.Prepare clear, professional clinical notes, progress reports, correspondence, treatment recommendations, and funding requests.Review provider reports and recommendations to ensure services remain coordinated and focused on the client’s goals.Maintain accurate documentation, service records, invoices, and time entries.Support clients using a trauma-informed, culturally responsive, and strengths-based approach.QualificationsBachelor’s or Master’s degree in Social Work.Registration and good standing with the applicable provincial College of Social Workers and Social Service Workers.Experience in rehabilitation, healthcare, mental health, community services, hospital discharge planning, disability management, or complex case coordination.Knowledge of acquired brain injury, spinal cord injury, chronic pain, orthopaedic injuries, psychological trauma, and cognitive or functional impairments is an asset.Strong assessment, counselling, advocacy, crisis-management, and problem-solving skills.Excellent written communication and clinical documentation abilities.Ability to work independently while collaborating effectively with an interdisciplinary treatment team.Strong organization and time-management skills, with the ability to coordinate multiple priorities and deadlines.Valid driver’s licence, reliable vehicle, and willingness to travel to clients’ homes and appointments as needed.Experience supporting individuals following motor vehicle accidents is an asset.The Ideal CandidateThe ideal candidate is empathetic without losing objectivity, confident when advocating for clients, and comfortable working with individuals and families during stressful and uncertain periods.
