MedicineOther medical specialtiesPhysicians will provide care within their respective scope of practice and specialty while collaborating with Avanta Health's clinical and operational teams to provide coordinated, efficient, and compassionate patient care.This position offers physicians an opportunity to build lasting patient relationships, collaborate across specialties, and contribute to the continued growth and development of Avanta Health's clinical services.Key ResponsibilitiesResponsibilities will vary depending on the physician's specialty and scope of practice and may include:Provide comprehensive evaluation, diagnosis, treatment, and ongoing management of patients within the physician's specialty.Perform medical histories, physical examinations, clinical assessments, and treatment planning.Develop individualized treatment plans based on patients' clinical needs, preferences, and goals.Provide preventive care, health screenings, patient education, and chronic disease management when appropriate.Evaluate and manage acute and chronic medical conditions within the physician's scope of practice.Prescribe medications and other appropriate therapies and monitor treatment effectiveness.Order, review, interpret, and appropriately follow up on laboratory testing, imaging, and other diagnostic studies.Maintain accurate, timely, and complete documentation within the electronic health record.Provide appropriate patient follow-up and promote continuity of care.Coordinate referrals and collaborate with primary care physicians, specialists, behavioral health professionals, nurses, and other members of the healthcare team.Collaborate with clinical and administrative staff to support efficient clinic operations and patient flow.Participate in quality improvement, care management, and population health initiatives as appropriate.Support accurate clinical documentation and applicable quality measures.Help patients navigate the healthcare system and facilitate appropriate referrals and follow-up care.Identify opportunities to improve clinical workflows, patient access, efficiency, and the overall patient experience.Maintain professional standards of conduct, medical ethics, patient confidentiality, and applicable regulatory requirements.Perform other related clinical duties consistent with the physician's specialty and scope of practice.Required QualificationsDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.Successful completion of an accredited residency program applicable to the physician's area of practice.Board Certified or Board Eligible in the physician's applicable specialty, where applicable.Current, unrestricted California physician license.Current DEA registration, if required for the position and scope of practice.Current BLS certification and other certifications appropriate to the physician's specialty and clinical responsibilities.Ability to practice independently within the physician's specialty and scope of practice.Candidates with the following qualifications are especially encouraged to apply:Vietnamese-speaking ability is highly preferred.Experience providing care to older adults and geriatric populations.Experience in outpatient, ambulatory, multispecialty, or community-based medical settings.Background in value-based care, managed care, population health, or similar healthcare environments.Strong clinical assessment and decision-making skills.Excellent communication and interpersonal skills.Ability to collaborate effectively across multiple medical specialties and disciplines.Commitment to preventive care, chronic disease management, patient education, and continuity of care.Demonstrated commitment to varies based on physician availability, specialty, patient demand, and clinic needs.The anticipated salary range for this position is $200,000–$800,000 per year.
