Entry level candidates are encouraged to apply!Compensation:$50,000–$55,000/yearAnnual salary increasesGrowth and advancement opportunitiesWhat You'll Do:Conduct community outreach and health screeningsSupport Medicaid members and individuals with asthma or chronic health conditionsComplete health assessments and questionnairesServe on home-visit teams of 2 (approximately 8 per month; some virtual)Connect community members with resources and health-related servicesAssist with environmental health initiatives and supply coordinationPresent educational workshops and community programsMaintain accurate records across multiple systemsRequirements:Fluent in Spanish and English (Required)Must live in Staten Island, NY or within a few mile radius.
