ResponsibilitiesReferral navigation: Leads warm-handoff referrals for program participants to BAART Programs (BayMark), local federally qualified health centers (FQHCs), and Sacramento County Behavioral Health, targeting 75 warm-handoff referrals over the project period.Follow-up and tracking: Maintains a referral tracking log and follows up with each referral within 2 to 3 weeks to confirm connection to care, working toward a 60% confirmed connection-to-care rate.Case coordination: Coordinates with partner clinics and community organizations to remove barriers to treatment, including transportation and language access.Reporting: Documents referral outcomes for quarterly UCLA-Integrated Substance Abuse Programs (UCLA-ISAP) data reports and bi-annual narrative reports.Community engagement: Supports trusted-messenger training sessions and community workshops as needed to identify individuals who may benefit from referral support.QualificationsEducation and training: Working knowledge of opioid use disorder (OUD), stimulant use disorder (StUD), and polysubstance use, and of the treatment and behavioral health referral landscape in Sacramento County.Communication: Strong interpersonal and case-management skills; ability to build trust with individuals affected by substance use and their families.Language: Proficiency in Dari and/or Pashto in addition to English strongly preferred, given the population served.Systems: Comfortable using digital tracking tools (Microsoft Forms, Google Sheets) to log referrals and outcomes.
