OverviewUnleashing Leaders is seeking a Transformative Care Model Clinical Adoption Support Lead to help drive meaningful healthcare transformation.This is an exciting opportunity for an experienced licensed healthcare professional who understands the realities of clinical environments and is passionate about helping healthcare organizations successfully adopt new care models, workflows, and transformational initiatives.As the Clinical Adoption Support Lead, you will play a key role partnering with healthcare providers, clinicians, health systems, community organizations, and other stakeholders to support sustainable clinical transformation. You’ll bring together clinical expertise, change leadership, stakeholder engagement, training, and data-driven improvement to help organizations move from strategy to successful adoption.Compensation & BenefitsSalary range: $160,000-$200,000/yearRemote/Hybrid opportunity and we are seeking individuals in the Sacramento, CA area

Medical, dental, vision, and life insurance, 401k company match, profit sharing, ESOP, PTO and sick time, and paid holidaysWhat You’ll DoDevelop and execute adoption strategies that support clinical and healthcare transformation initiatives.Build trusted relationships with providers, clinicians, community organizations, health systems, and other stakeholders.Assess organizational readiness and develop adoption plans tailored to different rural healthcare organizations and program needs.Serve as a subject matter expert and resource to grantee implementation teams.Develop communication strategies, toolkits, and resources that help healthcare organizations build awareness, alignment, and adoption.Facilitate workshops, forums, and training sessions for clinical and operational audiences.Partner with clinical and transformation leaders to align initiatives with the unique needs of rural healthcare environments.Help organizations translate complex clinical initiatives into clear workflows, tools, training, and implementation plans.Monitor adoption metrics, identify barriers to change, and develop strategies to address them.Establish KPIs and use data-driven insights to continuously improve adoption strategies.Document and share best practices to support learning and adoption across participating organizations.Support program leadership with reporting on adoption progress, risks, and opportunities.What We’re Looking ForBachelor’s degree in nursing, public health, healthcare administration, or a healthcare-related field—or an equivalent combination of education and relevant healthcare experience.5+ years of healthcare experience, including experience working with or alongside clinical teams and healthcare delivery organizations.Strong understanding of clinical operations, provider workflows, and the realities of implementing change in healthcare settings.Experience with clinical transformation, practice transformation, quality improvement, implementation, or adoption of new care delivery models.Ability to translate clinical concepts and evidence-based practices into practical workflows, tools, training, and implementation approaches.Demonstrated ability to build credibility and collaborate effectively with physicians, nurses, advanced practice providers, operational leaders, and other healthcare professionals.Preferred ExperienceClinical degree, credential, or licensure such as RN, NP, PA, MD/DO, or another relevant clinical credential.Experience working in rural health, safety-net settings, FQHCs, critical access hospitals, or underserved communities.Familiarity with primary care, maternal health, chronic disease management, behavioral health, or care coordination.Experience with technology-enabled care delivery, including telehealth, eConsult, or remote patient monitoring.
