Job Title: Value-Based Care Insights Coordinator

Location: Gilbert, AZ (on-site) or remote for candidates outside the Phoenix/Gilbert area.Compensation: $65,000-90,000 USD AnnuallyJob Summary:The Value-Based Care Insights Coordinator supports the organization’s Medicare value-based care initiatives across post-acute care and primary care environments.

This role is responsible for managing, analyzing, and reporting clinical and operational data that drives performance under value-based care models. The position requires advanced Excel expertise, strong communication skills, and the ability to think critically across complex care settings. The ideal candidate has hands on experience supporting value-based care workflows in post-acute and or primary care and understands how data informs quality, utilization, and outcomes.Key ResponsibilitiesCollect, validate, and maintain data supporting Medicare value-based care initiatives across post-acute care and primary care settingsDevelop and manage advanced Excel tools including formulas, pivot tables, lookups, and data reconciliation workflowsAnalyze performance trends related to quality measures, utilization, care coordination, and outcomesIdentify gaps, risks, and opportunities within value-based care performance and escalate findings appropriatelyPrepare recurring and ad hoc reports for leadership, clinical teams, and operational partnersCollaborate with clinical, operations, finance, and population health teams to ensure data accuracy and alignmentSupport chart reviews, audits, and data submissions related to Medicare value-based care programsEnsure data integrity and compliance with internal standards and Medicare reporting requirementsDocument reporting processes and contribute to continuous improvement initiativesCommunicate data insights clearly to both clinical and non-clinical stakeholders Required QualificationsMinimum of 2 years of experience supporting Medicare value-based care initiativesAdvanced Excel proficiency including complex formulas, pivot tables, and data analysisStrong analytical and critical thinking skills with high attention to detailExcellent written and verbal communication skillsAbility to manage multiple priorities in a fast-paced healthcare environmentExperience working with clinical or operational data in post-acute care or primary care settingsPreferred QualificationsExperience supporting value-based care models within post-acute care, primary care, or integrated care environmentsFamiliarity with quality measures, care coordination metrics, and utilization managementExperience working with population health, care management, or clinical operations teams
