We are a globally awarded clinic that designs and delivers value-based exercise medicine models of care, where success is measured by the outcomes patients actually experience. From our clinic in Cottesloe and our online virtual care clinic, we help people live well with chronic and complex conditions. Now we’re taking our next big step. CCA is the first provider in Western Australia to work with FITMED, and we are launching a dedicated exercise oncology service built on our value-based model. The service brings together exercise physiologists, physiotherapists and specialist dietitians, FITMED’s new clinical platform and the wider cancer care team around each patient. Our goal is simple: make expert exercise care a standard part of the cancer journey for everyone who can benefit. For the right clinician, this is a rare chance to be there from day one, helping shape a service that could change how cancer care is delivered in Western Australia. Why this role is different
- Be at the front door of care. You’ll triage clients as they enter the service and manage their care through the value-based model. Working with physiotherapists and oncologists, you’ll shape each person’s pathway from first contact to discharge.
- Build something, don’t just join something. You’ll be one of the first clinicians in a new value-based care model, with genuine input into how the service, programs and referral pathways take shape.
- Your outcomes count. Value-based care is about results that matter to patients. You’ll measure, track and show the difference exercise makes, and see that evidence shape the service.
- Help write best practice. You’ll be part of real-world evidence and research that will help develop a best-practice clinical pathway for exercise medicine in value-based healthcare. Working alongside physiotherapists and specialist dietitians, this is a rare opportunity to make a significant impact on your career.
- Be first in WA. CCA is the first provider in Western Australia to work with FITMED. You’ll be trained in its new clinical platform, one part of this model, to deliver and track care across our Cottesloe clinic and virtual care clinic.
- Become an exercise oncology expert. This is a real pathway to build specialist-level knowledge in one of the fastest-growing areas of exercise physiology. What you’ll do
- Triage new referrals, assessing each client’s needs, risks and goals to decide the right pathway and level of care within the value-based model
- Manage each client’s care across the pathway, coordinating with physiotherapists on rehabilitation needs and with oncologists on treatment plans, progress and any changes in medical status
- Assess clients and design individualised exercise programs for people at every stage of cancer care: prehabilitation, active treatment, recovery and survivorship
- Own your caseload, from first assessment and goal setting through progress reviews, outcome measurement and discharge
- Run individual and group sessions in clinic and via telehealth, adapting to treatment side effects such as fatigue, neuropathy, lymphoedema risk and reduced bone health
- Use sharp clinical reasoning and safety screening to know when to push, modify, pause or escalate
- Use FITMED’s clinical platform for documentation, program delivery and outcome tracking
- Contribute to real-world evidence and research by collecting high-quality outcome data and helping develop a best-practice clinical pathway
- Work in a multidisciplinary team with physiotherapists and specialist dietitians, alongside oncologists, GPs and nurses
- Keep accurate, timely clinical records in line with ESSA, Medicare, DVA, NDIS and private health insurer requirements as applicable
- Bring your ideas to service development, including program design, referral pathways and how the platform is usedAbout youYou’ve built solid experience as an AEP and you’re ready for work that stretches you.
